Comments:
The licensing inspector conducted an unannounced renewal inspection on 09/03/2019 from 10:35am to 1:30pm. The children were playing with play dough upon the inspector?s arrival, and they were later observed playing in centers, singing, playing games and eating lunch. Today, there were two children, totaling 6 points, present with one caregiver. The caregiver was observed interacting with the children. Interviews were held with the caregiver throughout the inspection. Additionally, when appropriate the inspector interacted with children. Lunch consisted of beef and macaroni, squash, applesauce, whole grain bread and milk.

The provider does not administer medication.
The provider does not provide transportation.
The provider does not have a pet.

Four children?s records, one household member and two caregiver/household member's records were reviewed during the inspection.

The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection.


The Department has not yet received a complete renewal application and annual fee from the provider. A complete renewal application and fee must be received by the Department prior to expiration of the license on October 22, 2019 or the license cannot be renewed.

Description: Based on record review, the provider did not ensure that one of three household members had a child protective services central registry check within 30 days of becoming 14 years old.
Evidence:
The record of household member #3 did not contain documentation of a required central registry check within 30 days of becoming 14 years old.

Plan of Correction: I will complete and submit the form within 10 days.
