An unannounced Subsidy Health and Safety Inspection (SHSI) Inspection was conducted on September 10, 2019 from approximately 10:30 am until 1:30 pm. Upon entrance there were 8 children, totaling 21 points and 2 caregivers. The record review sample consisted of 5 children's records, 1 provider record, 1 caregiver record and 2 household members records. Children were observed freely playing, eating lunch and during their rest period.
A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover the Subsidy Health and Safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
The results of the inspection were reviewed and verified with the provider during the exit interview. Areas of noncompliance are noted on the violation notice. If you have any questions, contact your licensing inspector at (757) 404-0568.

Standard #: 22VAC40-665-190-A Description: Based on record review, the provider did not ensure each caregiver must be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department .
Evidence: The record for caregiver #1 did not include a TB screening conducted within the required timeframe. The documented TB screening available for review was conducted in May 2018 and this was verified by caregiver #1.Plan of Correction: Caregiver #1 will get a new TB test/ screening.
