You are reminded that children under the age of 2 years must have updated immunization documentation obtained every six months.

Comments:
An unannounced Renewal inspection was conducted this date. A total of 12 children
(30 pts) were present with Provider and 2 assistants. When the inspector arrived, the older children were seated at the table practicing writing the letter "B" and coloring the letter. The younger children were free playing in the large programming area. This area is age appropriate for the younger children Toys, books and equipment were all within reach of the children to encourage self selection and independence. A nice transition was observed from writing at the table to observing a sign language video with staff. The children were engaged in this activity and were familiar with signing. Observation of the crib area, diapering area, bathroom and all three levels of the home was made. Hand washing procedures were observed prior to eating lunch. Lunch today was a homemade dish with chicken and rice and spice. Ample amounts for second serving which the children requested. Staff observed encouraging those old enough to eat with utensils to use them. Sliced oranges were served after their lunch. The tables and high chair were cleaned and sanitized by staff after lunch. Transition to nap time was made. Staff attentive to children's needs while transitioning to sleep. A total of 5 children's records, staff and household records were reviewed. No medications or food allergies at this time. This inspection began at approximately 10am and ended at 1pm. Thank you for your cooperation today. Standard #: 22VAC40-111-750-A
Description: Based on observation of the diapering area, authorizations for topical ointments were not obtained.
Evidence: Topical ointments were maintained for Child # 3 and #4. Parent authorizations for the use of these topicals were not obtained.

Plan of Correction: I will have this parent complete the topical ointment form today at pick up.

Standard #: 22VAC40-111-100-A
Description: Based on child record review, a physical examination was not obtained within 30 days of enrollment.
Evidence: Child #4 began care on 9/17/18. Documentation of a physical exam was not found. This was cited during the last two licensing inspections.

Plan of Correction: Updated immunizations were obtained for this child. I will ask for the physical exam from the parent and maintain in the record.

Standard #: 22VAC40-111-90-A
Description: Based on child record review, immunizations for one child were not obtained prior to enrollment.
Evidence: Child #3 began care on 9/3/19. Documentation of immunizations was not obtained. .

Plan of Correction: I called the parent during the inspection and requested a copy of the immunizations. Parent will provide to me and the documentation will be maintained in the record.

Standard #: 22VAC40-111-210-A
Description: Based on staff record review, annual training was not obtained.
Evidence: Staff #1 did not obtain a minimum of 16 hours of annual training.

Plan of Correction: I will enroll in training this week and I will complete the required 16 hours by the end of this year.
