Comments:
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 5 children, totaling 12 points. Children's records, caregiver/household records, caregiver training, administrative documents, emergency preparedness items, the physical plant, children's nutrition, background checks, and child care programming were inspected today.

The children were observed playing with toys, singing and dancing to music, and preparing for lunch. The children were served cheese pizza, peas, sliced grapes, and milk for lunch today.

Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observations, it was determined that one infant bottle of prepared formula was not labeled as required.
Evidence:
One bottle of formula was not labeled with the child's name.

Plan of Correction: I will remember to keep all infant bottles labeled.

Standard #: 22VAC40-111-120-B
Description: Based on a review of caregiver records, it was determined that one record was not complete.
Evidence:
Assistant caregiver record A does not include a second reference check.

Plan of Correction: I will complete the reference check.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record was not complete.
Evidence:
Child record B does not include the address for one emergency contact person.

Plan of Correction: I will ask the parent for the information.

Standard #: 22VAC40-111-70-A
Description: Based on a review of administrative documents, it was determined that one parent has not been given all required written enrollment information.
Evidence:
Parent B has not been given the provider's written medication policy information.

Plan of Correction: I will ask the parent to sign the form.

Standard #: 22VAC40-191-40-D-1-b
Description: Based on a review of caregiver background checks, it was determined that one required background search has not been completed.
Evidence:
The provider does not have documentation to show that the Central Registry Background search for assistant caregiver A was completed.

Plan of Correction: The search request was returned because I filled it out incorrectly. I returned the search request again a couple of months ago, but haven't heard anything about it yet. I will follow up and submit another request if necessary. I will submit the documentation to the Inspector within 10 days.
