Comments:
An unannounced monitoring inspection was conducted today from 9:30 am to 11:30 am. There were 3 children (7 points) with one caregiver. The children were observed playing with floor toys, singing and working on their alphabets. The physical plant, emergency plans, children's records, caregiver's records, attendance logs and emergency supplies were inspected. Areas of non-compliance were cited on the Violation Notice. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of children's records, the provider failed to ensure that all enrolled children will have a physical by within 30 days after the first day of attendance.

Evidence:
Child #3's record did not have documentation of a physical 30 days after the first day of attendance.

Plan of Correction: I will get it from parent.

Standard #: 22VAC40-111-60-A
Description: Based on a review of children's records, the provider failed to ensure that enrolled children's records are complete and up to date.


Evidence:

1) Child #3's record does not have documentation of an address for the second emergency contact.
2) Child #3's record does not have documentation of names of persons other than the custodial parents who are authorized to pick up child.
3) Child #5's record does not have documentation of an addresses for both emergency contacts.
4) Child #5's record does not have documentation of the name and policy number of the child's medical insurance.

Plan of Correction: I will get from the parents.

Standard #: 22VAC40-111-90-A
Description: Based on a review of children's records, the provider failed to obtain documentation that the child has been immunized by the first day of attendance.

Evidence: Child #3's record does not have documentation of immunizations on the first day of attendance.

Plan of Correction: I will get it from parent.

Standard #: 22VAC40-111-180-A
Description: Based on a review of household member's records, the provider failed to obtain for each adult household member a current report of Tuberculosis Screening form.

Evidence:
Household member #1's record does not have documentation of a current Tuberculosis Screening Report.
(last TB screening 5/25/17)

Plan of Correction: I will have household member submit TB Screening Report.
