Technical Assistance:
The inspector reviewed the following areas of FDH standards:

Comments:
An announced initial inspection was conducted on 10/03/2019 from 10:05 am - 12:30 pm. At the time of entrance there were three children in care with the provider. The ages of the children counted for a total of nine points. Children were observed during morning activity time which included a walk to the neighborhood playground. Lunch and nap was additionally reviewed.
The inspector reviewed the entire home which included the second floor of the home.
Records were reviewed for three children in care, the provider and two household members.
No medication is being administered.
Violations were observed in four parts of the FDH standards and were discussed with the provider at the conclusion of the inspection. These violations are listed on the violation notice.

Violations:
Standard #: 22VAC40-111-770
Description: Based on observation, the provider failed to ensure that there was a weather band radio available.

Evidence:
The battery operated radio was not a weather band radio.

Plan of Correction: A weather band radio will be obtained.

Standard #: 22VAC40-111-320-A
Description: Based on observation, the provider failed to ensure that small electrical appliances such as, but not limited to, curling irons, toasters, blenders, can openers, and irons shall be unplugged unless being used by the caregiver or with children under close supervision.

Evidence:
1. The toaster was observed plugged into the electrical wall outlet in the kitchen.
a. The toaster was not in use during the inspection.

Plan of Correction: The toaster will be unplugged when not in use.

Standard #: 22VAC40-111-330-A
Description: Based on observation and interview, the provider failed to ensure that a landline telephone shall be operable.

Evidence:
1. The land line telephone was not operable during the inspection.
2. The provider indicated that the telephone company had been to the home the previous day and were not able to install the telephone line.

Plan of Correction: The telephone company representative is scheduled to arrive later today for a second attempt to install an operable telephone line.

Standard #: 22VAC40-111-530-B
Description: Based on observation, the provider failed to ensure that cribs being used by infants shall have a tight-fitting bottom cover.

Evidence:
1. The inspector observed child 1, age four months, napping in a crib with a loose fitting crib sheet.
a. Child 1 has been placed face down in the crib at approximately 11:30 am.

Plan of Correction: A tight fitting crib sheet will be placed on the crib mattress.

Standard #: 22VAC40-111-590-A
Description: Based on observation, the provider failed to ensure that infants shall be placed on their backs when sleeping or napping unless otherwise ordered by a written statement signed by the child's physician.

Evidence:
The inspector observed the provider place child 1, age 4 months, in the crib on her tummy for nap at approximately 11:30 am.

Plan of Correction: Going forward all infants will be placed in the crib on their back.
