Comments:
A Subsidy Health and Safety inspection was conducted today to review supplemental health and safety requirements. The provider was caring for 2 children, totaling 6 points.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on a review of the provider's emergency preparedness plan, it was determined that this plan does not include all required components.
Evidence:
The emergency preparedness plan does not include a statement indicating a Reunification Plan.

Plan of Correction: I will create a reunification plan and add it to the emergency plan.
