Comments:
An unannounced monitoring inspection was on 10/16/2019 from 1:20 p.m to 2:45 p.m. to review family day home licensing standards. There were 6 children present in care. Children ranged from 14 months to 3 years old and a set of Household Twins 4 months old were also present with the provider and her assistant. The records sample size consisted of 4 provider / assistant / household member records and 6 children?s records. Children were observed engaged in quiet activities and sleeping on cots during rest time. The provider stated that she does not administer medication to children in care at this time and that there have been no accidents or injuries incurred by children in care requiring first aid or medical care since the last inspection. Description: Based on a review of records, the provider did not ensure that each enrolled child's record contained all of the required components.
Evidence:
Six children's records were reviewed.
The record for child #5 did not contain an address for one of the two emergency persons to be contacted in an emergency of the child's parents cannot be reached.

Plan of Correction: The provider stated that she will ask child #5's parents for this information and add it to the record this week.
