Comments:
A monitoring inspection was conducted on 10/29/2019 from 12:40 p.m to 2:00 p.m. to review the standards for licensed family day homes.
There were 4 children present, including one household child, for a total of 17 points, in care with the provider. The records sample size consisted of 4 provider / household member records and 3 children?s records. Children were observed cleaning up from play and transitioning to mats for rest period. The provider states that no medication is administered to children in care at this time. Children are sometimes transported in a vehicle and the requirements regarding transportation and outings were reviewed. Children play outdoors at a nearby public park with direct supervision and close proximity of the provider. The provider reports that there have been no accidents or injuries requiring first aid or medical treatment incurred by children in care since the last inspection. Areas of non-compliance are identified on the violation notice. Thank you for your time during today's inspection. Please do not hesitate to contact me if I may be of assistance.

Description: Based on a review of records and interview, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form,every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
The most recent available TB Screening for Provider / Household members #1 and #2 were dated 6/18/13 and 8/1/16 which is has been more than 2 years ago.
The provider stated that she did not have a more recently documented screening available.

Plan of Correction: The provider stated that she and household member #2 would both have updated screenings performed within 30 days and place the results in their files.
