Comments:
An unannounced monitoring inspection was conducted today from 8:50am to 11:30am with the provider. There were four children in care, ranging from 21 months to four years of age, for a total of 9 points. The provider shared that five other children attend before and after public school. Children were observed watching a children's television show, eating a morning snack of cereal and milk, and reviewing flashcards of animals. Six children?s records, the provider's record, and one household members? record were reviewed and were in compliance. The provider has current certification in CPR and First Aid. The provider shared that she will be getting certified in Medication Administration Training (MAT) soon, and she is aware that she is not able to accept or administer medication until she is certified. The first aid kit, flashlight, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Standard #: 22VAC40-111-760-A
Description: Based on observation and interview with the provider, the family day home did not have all of the required emergency supplies. Evidence: The provider was not able to locate adhesive tape or triangular bandages.

Plan of Correction: I will purchase these items.

Standard #: 22VAC40-111-770
Description: Based on interview with the provider, not all required emergency equipment was maintained on site. Evidence: The provider stated that she was unaware that she needed a battery-operated weather band radio.

Plan of Correction: I will purchase a battery-operated weather band radio.

Standard #: 22VAC40-111-830-A
Description: Based on documentation review and interview with the provider, evacuation procedures were not practiced monthly. Evidence: The emergency drill log did not show documentation of evacuation drills in the months of June and September of 2019 and the provider stated that she forgot to do evacuation drills those months.

Plan of Correction: I will make a reminder in my phone to help me remember moving forward.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous, substances such as disinfectants, were not stored in areas inaccessible to children. Evidence: There was a container of antibacterial wipes on the counter in the bathroom used by the children and was accessible to the children.

Plan of Correction: The container was moved during the inspection to the locked cabinet under the sink.

Standard #: 22VAC40-111-430-A
Description: Based on observation and interview with the provider, children under two years of age had access to stairs. Evidence: During the inspection, there was no gate at the top or bottom of seven stairs going to the upstairs and six stairs going to the downstairs. On this date there was a child present in care who is 21 months old.

Plan of Correction: I had a gate but it broke and a gentleman is going to come fix it for me.
