Comments:
An unannounced monitoring inspection was conducted on March 20, 2013. Children were observed during free play, circle time and lunch preparation. Licensing Inspector reviewed supervision, activities, equipment, hand washing procedures, diapering, special feedings, fire and health inspection reports, children and staff records, emergency evacuation and shelter-in-place drill documentation, emergency supplies and required postings. Please fax proof that you have secured cribs that meet current federal crib requirements. If documentation is not received within 30 days, the non-compliance will be reported to the Consumer Product Safety Commission (CPSC). Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed copy to the licensing office within 10 calendar days of receipt. There was a total of thirteen cribs in the younger infant and older infant classrooms that do not meet federal crib requirements as set by the Consumer Product Safety Commission (CPSC). 2. There was a metal pipe in the middle of a little tikes picnic table on the playground that was rusted and had sharp edges.

Description: Based on observation, facility did not ensure the surface used for diapering was non-absorbent. Evidence: On March 20, 2013, Licensing Inspector observed the diapering pad in the toddler classroom. The pad had multiple tears leaving exposed foam causing the surface to be absorbent.
