An announced initial inspection was conducted on October 31, 2019 to review subsidy health and safety inspection requirements. No children were present. Two children are currently enrolled. Provider record and two children records were reviewed.

Standard #: 22VAC40-665-170-A Description: Based on review, provider did not ensure 1 of 2 children's records has documentation of required information.
Evidence: The record for child #2 did not have father's home address.Plan of Correction: Provider will call the parent and ask her to bring the information today.
Standard #: 22VAC40-665-250-A Description: Based on observation, provider did not ensure potentially poisonous substances are kept inaccessible to children.
Evidence: Toilet bowl cleaner and calking (labeled with caution/danger and keep out of reach of children) were stored in an unlocked cabinet under the sink in the hall bathroom used by children.Plan of Correction: Items were moved during inspection.
Standard #: 22VAC40-665-400-A Description: Based on review, provider did not ensure there is a written emergency preparedness plan that addresses all required components.
Evidence: The emergency preparedness plan did not address procedures for shelter-in-place, lockdown, relocation, accommodations for infants/toddlers/special needs children and continuity of operations.Plan of Correction: Provider will complete the model form will all required information.
