Technical Assistance:
Discussed with staff background checks, documenting first day of employment and enrollments.

Comments:
An unannounced monitoring inspection was conducted from 7:30 am to 9:15am. There were 20 children in care of 2 staff members. The children were observed engaged in small group activities and leaving for the school day. 5 children's and 2 staff records were reviewed. Medication and medication forms were reviewed in addition to the first aid kit. Areas of non-compliance are listed in the violation notice. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review and staff interview, the center did not obtain documentation of a negative tuberculosis (TB) screening for an employee. Evidence: Staff # 2 stated that Staff # 1's first day of employment was towards the end of January 2019. Staff # 2 stated that Staff # 1 did not get a TB screening done.

Plan of Correction: I will get the TB results for the staff member.

Standard #: 22VAC40-185-70-A
Description: Based on record review, the first day of employment was not documented in staff records. Evidence: Staff # 1's record did not have the first day of employment documented.

Plan of Correction: I will document the first day of employment.

Standard #: 22VAC40-185-550-D
Description: Based on record review and staff interview, the center did not practice monthly evacuation drills and a minimum of two shleter in place per year. Evidence: 1. The fire drill log indicated that the last fire drill was conducted in November 2018. Staff # 2 stated that an evacuation drill was not conducted after that date. 2. There was no record of a shelter in place drill being practiced. Staff # 2 stated that she does no remember practicing a shelter in place drill in 2018.

Plan of Correction: Drills will be practiced as needed and records will be maintained.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and staff interview, the center did not obtain central registry findings for an employee within 30 days of employment. Evidence: 1. Staff # 2 stated that Staff # 1 approximate first day of employment was end of January 2019. 2. Staff # 3 stated that Staff # 1's central registry was sent out on 2/28/19. Staff # 1's central registry form was not mailed within 7 days of employment. 3. Staff # 3 was unable to demonstrate that a follow up was conducted to inquire about the results. 4. Staff # 4 was hired on 4/16/18. Staff # 3 stated that the form was resent multiple times due to errors or incomplete information. However, Staff # 4 was unable to show any documentation of the first time the paperwork was returned.

Plan of Correction: A follow up will be done and the licensing inspector will be contacted as soon as the results are obtained. Staff # 4's central registry will be resubmitted.
