An unannounced monitoring inspection took place on 04/25/2019. There were 9 children present and approximately 3 staff. The Inspector reviewed 5 children?s records, 5 staff's records and 3 medications. The children were observed in the following activities: free choice classroom time. An exit interview took place at the end of the inspection to discuss non-compliance. The Inspector arrived at 8:00 a.m. and departed at11:00 a.m.

Standard #: 22VAC40-185-520-B Description: Based on review of authorization, the center staff failed to document according to the standards. Evidence: The adverse reactions located on the sunscreen authorization form were not listed for Child 6. Plan of Correction: The center staff will obtain the documentation for the sunscreen authorization form for Child 6.
