Comments:
An unannounced monitoring inspection was conducted on May 13, 2019 from approximately 1:30 pm to 4:20 pm. Three staff were present with ten children. Five children's records and five staff records were reviewed.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review, facility did not ensure 1 of 5 staff have a completed TB test or screening within 21 days of beginning employment. Evidence: On May 13, 2019, the record for staff #4 (start date November 18, 2018) did not have a TB test or screening.

Plan of Correction: Staff will have the test/screening done tomorrow.

Standard #: 22VAC40-185-70-A
Description: Based on review, facility did not ensure 1 of 5 staff records have documentation of required information. Evidence: On May 13, 2019, the record for staff #1 (start date January 3, 2019) does not have documentation of the signature of the individual conducting reference checks over the phone.

Plan of Correction: Director will ask Head of School to sign for conducting the phone references.

Standard #: 22VAC40-191-60-B
Description: Based on review, facility did not ensure 1 of 5 staff have a completed sworn statement or affirmation prior to beginning employment. Evidence: On May 13, 2019, the record for staff #5 (start date June 11, 2018) did not have a completed sworn statement or affirmation.

Plan of Correction: Staff will complete the form tomorrow.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review, facility did not ensure 2 of 5 staff have a Office of Background Investigations (OBI) letter of eligibility for hire prior to beginning employment. Evidence: On May 13, 2019, the following was found: 1. The record for staff #1 (start date January 3, 2019) had a OBI letter of eligibility dated January 10, 2019. 2. The record for staff #5 (start date June 11, 2018) had a OBI letter of eligibility dated July 18, 2019.

Plan of Correction: Individuals will not begin employment until the letter is received.
