Comments:
The licensing inspector conducted an unannounced renewal inspection on 05/15/2019 from 9:05am to 2:05pm. The children were observed playing with toys, participating in art projects, reviewing numbers, reviewing seasons and eating morning snack. A formal observation was completed with the Three year old and Two year old classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Five children?s records and five employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. Eleven injury reports were reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 04/19/2019 Last shelter-in-place drill: 03/11/2019 Last health inspection: 05/09/19 Last fire inspection: 12/19/2018 Today, the following child to staff ratios were observed: Infants ? 7:2 Toddlers ? 5:2 Twos ? 12:2 Threes ? 8:1 Fours - 7:1 The violations from the previous inspections were checked for corrections. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review, the center did not ensure that one of five children's records contained documentation of a physical examination within one month after attendance. Evidence: The record of child #5 (start date: 9/10/18) did not contain documentation of a physical examination.

Plan of Correction: We will get the child's physical record by next Friday.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: The threes and twos classroom had unlocked cabinets containing cleaners out of the reach of children.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the center did not ensure that children's hands were washed with soap and running water after toileting and staff wash their hands with soap and running water before and after a diaper change. Evidence: The inspector observed a staff member diaper several two year old children consecutively without washing their hands before or after diapering. The staff member was observed changing disposable gloves in between diapering each child. The children's hands were not washed after receiving a diaper change.

Plan of Correction: We will ensure that staff wash their hands and the children's hands in between diapering.
