Comments:
This unannounced monitoring inspection was conducted from 10:35 A.M. until 2:10 P.M. At the time of the inspection 97 children were present with 14 staff. The sample size consisted of nine children's records, nine staff's records, and four children's medications. Children and staff were observed during free indoor play, outdoor play, snack, story time, educational activities, show and share, clean-up, bathroom break, hand washing, nap, transitions, and behavioral guidance. The violations from the previous inspection were reviewed for current compliance. Violations can be found on the violation notice.

Standard #: 22VAC40-185-160-A
Description: Based on record review and interview, the facility failed to have each staff member submit documentation of a negative tuberculosis (TB)screening submitted no later than 21 days after employment. Evidence: 1. The records for nine staff were reviewed. Staff 8's first date of employment was documented as 2/25/19. The record for staff 8 does not have documentation of a TB screening. 2. The director verified they do not have a TB screening for staff 8 and stated staff 8 was a sub that has not worked since 4/12/19 and has another job for the summer.

Plan of Correction: If staff 8 returns to work staff 8 will be required to provide documentation of a TB screening prior to returning. In the future all new staff will be required to provide documentation of a TB screening prior to the end of 21 days of employment.
