Comments:
A self reported incident was reported on April 23, 2019. The inspection was initiated on April 24, 2019 and concluded on May 24, 2019. A child sustained a broken arm when he fell from the platform of the large slide. The self report resulted in a violation due to the inadequate amount of resilient surfacing in the fall zone. *Please complete the plan of correction and date to be corrected for each violation cited on the Violation Notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).Please call me if you have any questions at 804-662-9766 or e-mail tara.barton@dss.virginia.gov.

Standard #: 22VAC40-185-330-B
Description: Based on observation and staff interviews, the facility failed to have adequate resilient surfacing as required in the fall zones underneath and surrounding equipment. Evidence: 1. On 4/24/2019, on the large playground equipment, the mulch in the fall zone of the large slide platform measured only 3 inches in depth. The platform of the slide is 5 feet 10 inches tall, so a minimum of six inches of resilient surfacing (mulch) is required. At the bottom of the slide, there was no mulch present, only bare ground. 2. On 4/23/2019, Child A (age 4) fell from the 5'10" platform located at the top of the slide onto the ground. Child A received emergency medical attention for a broken arm.
