Comments:
An unannounced complaint investigation was initiated on April 24, 2019 with center owner and center staff from 3:00 p.m. to 5:30 p.m. Additional interviews and review of documentation was conducted on May 24, 2019 from 3:20 p.m. to 5:30 p.m. Based on staff interviews, review of children's files, and review of the employee handbook, the allegation regarding a field trip without written permission from parents was determined to be valid. Based on observation, review of attendance records, and staff interviews, the allegation regarding ratios was determined to be valid. *Please complete the plan of correction and date to be corrected for each violation cited on the Violation Notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Description: Based on observation and staff interview, the facility failed to maintain the ratio applicable to the youngest child in the group when children are regularly in ongoing mixed age groups. The ratio is 1 staff for every 18 children for the younger school age group. Evidence: the younger and older school age children are combined in the school age classroom every day. On 4/24/2019 from 3:00 to 4:20 p.m., there were 36 school age children (ages 5 through 12) with Staff F. Staff G assisted in the classroom from 4:20 p.m. to 5:30 p.m., when the ratio reduced to eighteen children.

Description: Based on observation, review of the attendance record, and staff interviews, the facility failed to maintain the ratio of one staff person for every 10 children ages three years to age of five years by September 30. Evidence: 1. On April 24, 2019 from 8:30 a.m. to 10:00 a.m., there were reportedly 18 preschool children (ages 3 and 4 years) with one staff (Staff E). 2. On April 24, 2019, from 3:00 p.m. to 4:30 p.m., the licensing inspector observed 24 children (ages 3 and 4 years) with two staff (Staff C and Staff D). 3. When the children arrive from preschool at 3:00 p.m. each day, there are 14 to 18 four year old children present with one staff (Staff A). The youngest child on the roster (Child C) is currently 4 years, 6 months of age and is not eligible to attend public school by September 30.

Description: Based on review of written documents and staff interview, the facility did not ensure that when using a blanket permission form for transportation and field trips that parents were notified of a field trip and were given the opportunity to withdraw their child from the field trip Evidence: 1. On Friday, March 22, 2019, Staff A and Staff B took five children (age 4) on the center van to the bank and then to a local grocery store. 2. The center uses a blanket transportation permission slip where parents select that they grant permission, grant permission with prior notice, or deny permission for the center to transport the child to and from field trips. 3. The transportation permission slip for Child A and Child B granted permission for the center to transport the children to and from field trips with prior notice. 4. Per Staff A, the parents were not given prior notice of the field trip.
