Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, July 25, 2019. The on-site inspection was conducted from 10:40 a.m. to 1:15 p.m. On this date, at the time of arrival, there were 28 children present in two groups and in the care of three staff members. There was an additional staff member available to assist if needed. There were a total of 33 children enrolled in the elementary and primary summer camp programs. Observations were made in both groups. The exception to the licensing standards was noted on the violation notice and the risk rating was discussion. Five staff members' records and five children's records were reviewed. The medication and medication paperwork were reviewed for two children. The elementary group was studying Egypt and the primary group was studying the Alps. Some items observed and reviewed during the visit included injury reports, required posted information, documentation of emergency evacuation and shelter-in-place drills, medication authorizations, Montessori work activities, mummifying a real chicken, making play dough, floor games and activities, hand washing and bathroom procedures, lunch, nap time, and dismissal procedures. Please contact your licensing inspector, Diann Reed, if you have questions concerning this inspection, the licensing standards, and/or if I may be of assistance to you (540) 280-0742.

Violations:
Standard #: 22VAC40-185-510-C
Description: Based on review of two medication records and verification with the Head of School, the center failed to have parental and physician authorization for one medication. Evidence: 1. The paperwork for Child A's long-term emergency medication did not contain authorization from the child's parent or from the child's physician allowing the center to administer the medication if needed. The center originally had parent authorization to administer the medication but it expired on 07/09/2019.

Plan of Correction: The staff reached out to the parent during the inspection to request updated parental authorization and is asking that the child's physician also complete a long-term authorization.
