Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:00 am - 11:30 am. There were thirty one preschool children in care: seventeen children with two staff in classroom #1 and fourteen children with two staff in classroom #2. The children were observed coloring, at circle time, listening and moving to music, and in small group activities. One staff's record and five children's records were reviewed. Violations from the previous inspection were also reviewed and one was repeated. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain one outside play area in a safe condition.
Evidence:
1. The inspector observed the play area located directly behind the center. There is a broken landscape timber next to the fence that has sharp edges.

Plan of Correction: The landscape timber will be replaced.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurement, the center failed to have 6 inches of resilient surfacing on the playground.
Evidence:
1. The inspector observed the playground located behind the school. The resilient surfacing (pea gravel) measured 1 - 2 inches in the fall zone surrounding the mats at the base of the fort. The fall height of the fort is approximately 40 inches.

Plan of Correction: Pea gravel will be added to meet regulations of six inches.

Standard #: 22VAC40-185-550-D
Description: Based on record reviews, the center failed to document practiced evacuation drills.
Evidence:
1. The inspector reviewed the evacuation drills for 2019 in classroom #2. There were no documented drills for the months of January, April or May.

Plan of Correction: The teacher in classroom #2 said the evacuation drills were practiced, but were not documented. Teacher will document the fire drills.
