Emergency evacuation and shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of each building.

An unannounced monitoring inspection was conducted on September 10, 2019 from approximately 2:45 pm to 4:30pm. Three staff were present with forty-four children. Children were observed participating in station activities, homework and gym games. Five children's records were reviewed. Staff training and required information to be maintained on site were reviewed for the three staff present.

Standard #: 22VAC40-185-130-A Description: Based on record review and staff interview, the licensee did not ensure center obtain documentation that a child has received immunization before the child can attend the center.
Evidence:
The center did not have immunization for child #3 before attending the program. Staff #1 reviewed documentation and was unable to locate the immunization record.Plan of Correction: Immunization record for child #3was on file at central office but had not been upload into the system. The record was uploaded the morning of 9/11/2019.
Standard #: 22VAC40-185-550-D Description: Based on record review and staff interview, the licensee did not ensure monthly practice evacuation drill were documented.
Evidence:
The center did not have documentation that monthly evacuation drills were completed March-June.Plan of Correction: The Center Supervisor will ensure the monthly evacuation drill log is posted for quick reference.
