Comments:
An announced initial inspection was conducted on September 24, 2019, from approximately 9:10 AM to 12:30 PM. Space and equipment observed were appropriate for children ages infants- 12 years. The exit interview was conducted with the owner. The inspection will be reviewed by administrator for license issuance.

Violations:
Standard #: 22VAC40-185-330-B
Description: Based on observation and measurements, the applicant did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards, and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use.

Evidence;
The resilient surfacing on the playground was inadequate around the two playground composites. The resilient surfaces on the playground were not able to be measured for fall zone and resilient surface depth. The resilient surfacing was mulch.

The minimum requirement for the surfacing is 6 inches and minimum requirement for climbing apparatus is 6 feet fall zone and the fall zone for a slide is height plus four feet.

Plan of Correction: The Playground composites will not be used and caution tape will be put on the equipment until mulch is replaced. All other space will be used for outside game and activities.

Standard #: 22VAC40-185-380-A
Description: Based on observation and staff interview, the licensee did not ensure the daily schedule was posted.

Evidence: There was not a daily schedule posted in the preschool classroom , early preschool classroom and toddler classroom.

Plan of Correction: The daily routine were posted during the inspection.
