An unannounced complaint investigation was initiated on August 16, 2019 regarding supervision, rough handling of a child, and physical plant. Additional information was received September 5, 2019. Based on staff interviews and review of video footage, the allegation regarding inappropriate behavioral guidance of a child was determined to be valid. Standard #: 22VAC40-185-400-B Complaint related: Yes Description: Based on staff interview and video footage, the facility failed to provide behavioral guidance that is constructive and age appropriate. Evidence: an undated video recording in June 2019 or July 2019 shows Staff 1 carrying 4 year old Child 1 by left upper arm. The child is crying and his feet are not touching the floor. Staff 1 takes the child into an empty classroom and shuts the door. Plan of Correction: Not available online. Contact Inspector for more information.
