Comments:
An unannounced Monitoring Inspection and Self Reported Incident was conducted today from 3:15pm to 5:30pm. There were 68 Infants to 5 years of age directly supervised by 17 staff. The physical plant, 10 staff records, 10 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside and eating snack. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Description: Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: 1) Observed peeling paint in Room 2 and 5. 2) Observed 2 nails/screws protruding from the fence on the toddler playground. 3) Observed 2 nails/screws protruding from the fence on the large playground

Description: Based on observation, the facility did not ensure that electrical outlets have protective covers. Evidence: Observed in the Dance room, an uncovered electrical outlet above the counter.

Standard #: 22VAC40-185-400-B
Description: Based on staff interview, written statements and injury report, staff did not ensure that behavioral guidance is constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts. Evidence: Child A was "punching and throwing" the library couch when Staff A approached the child and held the child's hand, leading the child away from the library. The child pulled back from the teacher and the teacher "pulled the child up from the ground with (their) arm". The child instantly was holding their elbow and crying, and the staff member held the child.

Description: Based on observation, the facility did not ensure that materials and equipment available shall be age and stage appropriate for the children. Evidence: Observed in Rooms 6, 10, 9 and 7, staples used on the bulletin board, that was easily accessible to children and found that several staples were loose and easily removed if manipulated.
