The inspector conducted an unannounced mandated inspection from 10:30 a.m. until 12:30 p.m. There were 12 children in care with three staff present. Children were observed making crafts and playing with manipulatives and getting ready for lunch. Five children's records were reviewed, and staff records were reviewed. Four buses were inspected for both classrooms. No medications were reported being administered. Please contact your inspector at 276-698-9981 with any questions.

Standard #: 22VAC40-185-270-A Description: Based on observation, the center failed to maintain the center in a safe condition.
Evidence:
1. The inspector observed a broken wheel and missing petals on the multi-tryke play equipment.Plan of Correction: Children will be kept off the equipment until it can be removed.
Standard #: 22VAC40-185-330-B Description: Based on observation and measurement, the center did not meet the minimum safety standards for fall zones on the playground.
Evidence:
1. The inspector measured the fall zone for the gray climbing wall and there was 5 feet of use zone of mulch; the requirement is 6 feet on all sides for climbing equipment. 2. The inspector measured the mulch around the slide and the climbing wall and it was 2-3 inches thick;required depth is 6 inches.Plan of Correction: Mulch will be added and redistributed and fluffed. The goal is to have rubber mulch in the future.
