Comments:
The licensing inspector conducted an unannounced renewal inspection on Tuesday, November 19, 2019 between 11:00 am to 2:25 pm to determine the center's compliance with licensing standards. The center's assistant director and owner were present to accompany and assist the licensing inspector throughout this inspection. The licensing inspector completed observations of 114 children in care. During the inspection, the licensing inspector observed the children in care participating in a variety of activities to include asleep in cribs, engaged in active play on carpet, eating their lunches brought from home, participating in story time, completing bathroom breaks/handwashing, diapering by staff, transitioning from lunch time, and preparing for napping.The center's classrooms, bathrooms, diapering areas, indoor and outdoor areas were inspected. During this inspection, the required postings were reviewed and determined to be in compliance. The following information was reviewed: daily attendance, emergency supplies, injury reports, emergency drills, liability insurance policy. There were seven children?s records and six staff's records reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Description: Based on review of staff records and interview with staff, the center did not ensure to obtain all of the following information.

Evidence: In Staff#6's record (date of hire 09/03/19) the dates of contact was not found for two or more references. Interview with staff confirmed the information reviewed in the record.

Plan of Correction: The center will ensure all documents area dated the date conducted.

Standard #: 22VAC40-185-270-A
Description: Based on observation at the center, the center did not ensure all areas were maintained in a clean, safe, and operable condition.

Evidence: In the Turtles classroom, there was a red cushion located on the carpet observed torn in multiple areas with foam exposed which was accessible to the children in care and presented a safety concern due to ages and stages of this group.

Plan of Correction: The center director threw away during inspection.
