An unannounced monitoring inspection was conducted on November 22, 2019 from approximately 2:00pm- 4:30pm. There were 4 children and 3 staff present at the center . Children were observed resting, eating snack and going outside. Licensing Inspector reviewed indoor areas, supervision, activities, equipment, medication procedures, emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.
Exit interview was conducted with the director.

Standard #: 22VAC40-185-60-A Description: Based on review, facility did not ensure 1 of 5 children records has documentation of required information.
Evidence: The record for child #4 was missing one emergency contact. Staff confirmed information was not available.Plan of Correction: Will have parent named as the second emergency contact.
Standard #: 22VAC40-185-70-A Description: Based on review, facility did not ensure 1 of 3 staff records has documentation of required information.
Evidence: The record for staff #2 did not have two reference results and emergency contact information.Plan of Correction: The references will be completed by next Tuesday. Emergency contact information will be completed today.
Standard #: 22VAC40-185-270-A Description: Based on observation, the licensee did not ensure areas and equipment of the center, outside, shall be maintained in a clean, safe and operable condition.
Evidence:
Outside on the playground, there was a board that was cracked and splintering and a board with attached metal piece with sharp points. On the playground there was a hammer and a crow bar on the picnic table.
.Plan of Correction: The boards and tools will be removed.
Standard #: 22VAC40-185-560-F Description: Based on observation and staff interview, the licensee did not ensure when center choose to provide meals or snack the menu is dated.
Evidence: There was not a dated menu posted in the center.Plan of Correction: The date will be put on the menu.
Standard #: 22VAC40-191-60-B Description: Based on record review the licensee did not ensure a completed sworn statement or affirmation before they are employed or provide volunteer service.
Evidence:
The record for Staff #2 ( hire date 11/21/2019) did not have a sworn statement or affirmation.Plan of Correction: The director had the sworn statement of affirmation completed by staff #2 during the inspection.
