Comments:
An unannounced monitoring inspection was completed on 12/09/2019. There were 21 children, ages 4 years-10 years, and 3 staff members present during the inspection. The inspector reviewed 5 children's records, 3 staff records and 3 medications during the inspection. The children were observed in the following activities: organized large motor play in the gym, during afternoon snack, during homework time and in free choice activities. The inspector reviewed the emergency supplies during the inspection. The inspector discussed the medication requirements and playground requirements with the staff. The inspector arrived for the inspection at 3:20 p.m. and departed at 5:30 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Standard #: 22VAC40-185-290-3
Description: Based on observations and discussion with the staff, the facility failed to ensure that in areas used by children of preschool age or younger, electrical outlets shall have protective covers.

Evidence:
1. There were 11 uncovered electrical in the gymnasium; children were observed playing in the gym during the inspection. The staff verified that 4 year old children were playing in the gym during the inspection.
2. There was an uncovered electrical outlet in the cafeteria; children were observed in the cafeteria during the inspection. The staff verified that 4 year old children were in the cafeteria.

Plan of Correction: The outlets will be covered daily.

Standard #: 22VAC40-185-510-J
Description: Based on observations and discussion with the staff, the facility failed to ensure that medication shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: Medication A was located in a broken Safety Sack, which preventing the device from locking, inside of an unlocked first-aid back pack. The unlocked first-aid back pack was on the stage area of the gym and was accessible to children.

Plan of Correction: The Safety Sack was repaired during the inspection.
