Comments:
An unannounced monitoring inspection was conducted with center staff on December 11, 2019 to determine compliance with the licensing standards. There were 78 children present, ranging in age from 1 year to 12 years, with nine staff actively supervising. The preschool children were waking from rest, eating afternoon snack, having free play, and making holiday projects. The school age children were arriving from school, working on homework, and eating snack. The playground was not reviewed due to inclement weather. Five staff files and eight children's files were reviewed - please see Supplemental Summary for details. Seven medications for six children were reviewed. Violations:
Standard #: 22VAC40-185-430-L
Description: Based on observation and staff interview, the facility failed to prevent the use of an infant walker. Evidence: an infant walker was observed in the infant room. When asked, staff stated that the infants use it.

Plan of Correction: Center staff emailed on 12/13/2019 to state the wheels had been removed from the infant walker.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the facility failed to wash children's hands and staff hands with soap and running water after toileting/diapering. Evidence: After changing three toddler's diapers, Staff 2 failed to wash the children's hands with soap and running water. Staff 2 changed gloves, but did not wash her hands with soap and running water after each diaper change.

Plan of Correction: Center manager will retrain Staff 2 on hand washing requirements before and after diaper changes.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the facility failed to clean the diaper pad with soap and water, sanitize it with bleach and water or similar disinfectant, and allow to air dry for at least two minutes after each diaper change. Evidence: Staff 2 did not clean, sanitize, and air dry the diaper pad for at least two minutes after each diaper change when changing three toddlers.

Plan of Correction: Center manager will retrain Staff 2 on proper cleaning, sanitizing, and air drying after each diaper change.
