Comments:
The licensing inspector conducted an unannounced monitoring inspection that began at the center at 9:00 am and concluded with staff record reviews at the central office at 12:30 pm. There were eighteen preschool children in care with three staff and one volunteer. The children were observed at circle time, at story time and playing in the centers. Five children's records and three staff's records were reviewed. Violations from the previous inspection were also reviewed and had been corrected. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record reviews, the center failed to have a current tuberculosis screening on a staff.
Evidence:
1. The inspector reviewed three staff's records. One of three records reviewed (staff record #2) contained a tuberculosis screening that was over two years old. The most recent screening was dated 12/6/17.

Plan of Correction: The tuberculosis screening for staff #2 will be repeated. Staff has an appointment scheduled.

Standard #: 22VAC40-185-270-B
Description: Based on observation, the center failed to provide a barrier to an emergency heat source.
Evidence:
1. The inspector observed the center. There was an electrical fire place heater being used to heat the center. The grill in the heater was metal and very hot. There was no barrier around the heater.

Plan of Correction: The heater is being used because the heat pump is currently not operable. A barrier will be placed around the heater and the children will be supervised closely.

Standard #: 22VAC40-185-550-D
Description: Based on documentation review and interview, the center failed to conduct monthly fire evacuation drills.
Evidence:
1. The inspector reviewed the fire drill documentation and there were no drills documented for November and December 2019.
2. The inspector asked the teacher if drills were practiced during these months and the teacher said they had not.

Plan of Correction: The teacher said the center will practice two fire drills in January and two fire drills in February to make up for the missing drills.
