Comments:
Thank you for your time and assistance with the unannounced renewal inspection conducted today, February 19, 2020. The on-site inspection was from 11:50 a.m. to 4:55 p.m. On this date, at the time of arrival, there were nine children present in two classrooms and in the care of three staff members. There were 12 children enrolled in the program. Four staff members' records and five children?s records were reviewed. Observations were made in both classrooms. The medication and medication paperwork were reviewed for one child. There were four staff members and two children interviewed. The violations from the previous inspection were reviewed for compliance. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. The lunch served met the USDA guidelines and consisted of chicken tenders, sweet potatoes, peas, and milk. Some items observed and reviewed during the inspection included: transition from outdoor play, preparation for lunch, lunch, nap time, bathroom and handwashing procedures, arrival of school-age children, dismissal procedures, a turtle art activity, singing and dancing, infant care, diaper changing procedures, story time, documentation of evacuation drills, required posted information, injury reports, first-aid kit and emergency supplies, and warm water temperature.

Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records and confirmation from the director, the center failed to obtain documentation that one child had received a physical examination within one month of attending the center.

Evidence:
1. The record for Child #1 contained a physical examination report dated 12/12/2019. The date of enrollment for the child was 11/4/2019.
2. The director confirmed the physical examination report was not received within a month of the child being enrolled.

Plan of Correction: The director will ensure that a physical examination report is obtained either before the child attends the center or within a month of attending.
