An unannounced monitoring inspection took place on February 19, 2020. There were 79 children in care from ages 3 years old to 6 years old with 11 staff providing care and supervision. The inspector reviewed eight children's records and eight staff records onsite and on this day. The inspector assessed three medications onsite, which encompassed all medications for that site. Children were observed during the following activities: arrival, free play, snack, drawing activities, matching games, and story time. The inspector also reviewed emergency drill documentation, first aid kits, the physical plant, and injury/incident reports. There were discussions about topical substances and activated charcoal. The inspection took place from approximately 8:06 AM to 12:30 PM. Violations were discussed at the conclusion of the inspection. Standard #: 22VAC40-185-330-B Description: Based on observation and discussion with the staff, the facility failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards.
Evidence: The areas at the bottom of the slide and climbable equipment on the playground did not contain the required depth of resilient surfacing; dirt was visible near the equipment.Plan of Correction: Staff will create a set schedule for replenishing mulch.
