An unannounced monitoring inspection was conducted on February 27, 2020 from approximately 9:40 am to 12:00 pm. Two staff were present with twelve children. Five children records were reviewed. Training and emergency contact information were reviewed for the two staff present.

Standard #: 22VAC40-185-270-A Description: Based on observation, facility did not ensure the areas and equipment of the center are maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.
Evidence: The counter border at the children's sink is separating causing splintered wood and sharp points.Plan of Correction: Staff will put in a maintenance request to have it repaired.
Standard #: 22VAC40-185-550-M Description: Based on review, facility did not ensure required information is documented when a child sustains an injury.
Evidence: The record for child #2 had an accident/incident log form completed on October 17, 2019 that did not document parent notification (date, time and method), staff present, plan of preventing reoccurrence and two signatures.Plan of Correction: Staff will use an updated form when documenting injuries.
