Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted on February 20, 2020. The on-site inspection was from 10:25 a.m. to 1:50 p.m. On this date, there were 43 children present in five classrooms and in the care of ten staff members. There were three additional staff members available to assist if needed. There were 71 children enrolled in the center. Five staff members' complete records were reviewed and the background checks and tuberculosis screenings were reviewed for seven new staff members. The records for five children were reviewed. Five classrooms were toured and observations were made in three of those classrooms. Six staff members and four children were interviewed. The medication and medication paperwork were reviewed for one child. Lunch was observed to meet CACFP guidelines and consisted of chicken tenders, green beans, peaches, wheat bread, and milk. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. Some items reviewed/observed: outdoor play, transitions to and from the playground, infant feeding, infant care, story time, diaper changing procedures, bathroom and hand washing procedures, lunch, art activities, nap time, injury reports, documentation of evacuation drills, first aid kit and emergency supplies, warm water temperature, and required posted information.

Violations:
Standard #: 22VAC40-185-320-B
Description: Based on observation and confirmation from staff, the center failed to keep the temperature of the hot water below 120 degrees Fahrenheit in three classrooms.

Evidence:
1. Using a thermometer, the hot temperature was measured to be 131.3 degrees Fahrenheit and 30 minutes later was measured to be 124.1 degrees Fahrenheit in Classroom D. Staff Member A confirmed the reading of 124.1 degrees.
2. In Classroom C, the hot water temperature measured to be 121.3 degree Fahrenheit.
3. In Classroom E, the hot water temperature measured to be 124.4 degrees Fahrenheit. Staff Member B confirmed the reading of 124.4 degrees.

Plan of Correction: The administration will work to find the reason that the hot water temperature is higher in one area of the building. They will turn the temperature of the hot water heater down to ensure the water is not higher than 120 degrees.
