This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on May 7, 2020 and concluded on May 8, 2020. The director in-charge person was contacted by telephone to initiate the inspection. There were 24 children present and 7 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #: 22VAC40-191-60-C-2 Description: Based on review of staff record and interview with staff, the center did not ensure to obtain a central registry finding within 30 days of employment or volunteer service.
Evidence: In Staff#1's record (date of hire:1/23/2020) there was no central registry findings found for review. Interview with the staff confirmed the central registry findings for Staff#1 was not available for review during this inspection.Plan of Correction: Staff submitted a follow up letter to crs_operations for a follow up to the 02/06/2020 Central Registry request for Staff #1. In addition, staff called OBI/Central Registry number provided by LI and left a message regarding the registry request. Corrected immediately
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of staff record and interview, the center did ensure to obtain a copy of results of an out of state central registry search.
Evidence: In Staff#1's record (date of hire:1/23/2020), there was no results of an out of state central registry search found for review. Interview with staff confirmed a search of the out of state central registry for Staff#1 had not been requested at this time.Plan of Correction: Staff was able to find information regarding out of state central registry information. Paperwork must be submitted via mail with a check; they would not accept an online request. Paperwork completed today, 05/08/2020. Corrected immediately.
