This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on August 3, 2020 and concluded on August 19, 2020. The director person was contacted by telephone to initiate the inspection. There were 35 children present and 8 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children's records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-60-A Description: Based on record review and discussion with the staff, the facility failed to ensure that the separate record for each child enrolled shall contain all of the elements as required by the standards.
Evidence:
1. The record for Child #3 did not contain all required health information as required by 22VAC40-185-130 through 22VAC40-185-150 as described below.
A. The record for Child #3 contained one documented physical examination, which was dated 08/10/2017. The only documentation of immunization is dated 08/10/2017. The child's recorded date of enrollment was 09/06/2016.
