Comments:
A monitoring inspection was initiated on April 21, 2022 and concluded on April 21, 2022. There were 60 children present, ranging in ages from Infant to Preschool, with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of staff records, the center did not ensure that each staff member shall submit documentation of a negative tuberculosis screening (TB) at the time of employment and prior to coming in contact with children.

Evidence:
1. The record for staff #1 (start date: 4/18/22) and staff #3 (start date: 3/03/22) did not have documentation of the TB screening or test at time of employment.

Plan of Correction: Will not allow teachers to start till TB is read.

Standard #: 8VAC20-780-160-C
Description: Based on a review of staff records, the center did not ensure that subsequent communicable tuberculosis (TB) screenings were conducted at least every two years from the date of the initial screening.

Evidence:
2. The record for staff #5 (date of last TB screening 12/02/19) did not have an updated TB screening/test.

Plan of Correction: Has had updated TB as of 5/02/22

Standard #: 8VAC20-780-280-B
Description: Based on observation and an interview, the center did not ensure hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In classroom #3, a container of Glade fragrance spray labeled "Harmful" and "Keep Out of Reach of Children" and a container of disinfectant spray labeled "Harmful" and "Hazards to Humans? in a unlocked cabinet.
2. In classroom #5, a container of multi-surface cleaner labeled "Hazards to Humans" and "Keep Out of Reach of Children" and one container of Peroxide labeled "Harmful? and "Keep Out of Reach of Children" was in an unlocked cabinet.
3. In classroom #7: a container of Fabuloso labeled ?Caution? and "Keep Out of Reach of Children,? Lysol disinfectant wipes labeled ?Caution? and "Keep Out of Reach of Children,? Glade fragrance spray labeled "Harmful" and "Keep Out of Reach of Children,? a container of Resolve labeled "Keep Out of Reach of Children" and ?Caution,? and a container of glass cleaner labeled ?Caution? and "Keep Out of Reach of Children" in an unlocked closet located in the children?s bathroom.

Plan of Correction: All sprays were removed from classroom.

Standard #: 8VAC20-780-510-E
Description: Based on observation and staff interview, the licensee did not ensure medication shall be labeled with the child?s name, the name of the medication, the dosage amount, and the time or times to be given.

Evidence:
One medication, Albuterol was observed without the prescription label, child?s name, the dosage amount, and the time or times to be given. Center Director confirmed that the medication did not have a label with the required information.

Plan of Correction: Parents will be asked to bring a new box with label.
