A monitoring inspection was initiated on 09/14/2022 and concluded on 09/14/2022. There
were twenty children present, ranging in ages from 5 months to 4 years, with nine staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of seven child records and eight staff records were reviewed.

Standard #: 8VAC20-780-160-A Description: Based on record review, center failed to ensure a staff member received documentation of a negative tuberculosis screening.
Evidence:
1. Staff #7 date of hire was 07/05/2020 during the COVID pandemic. The leniency for TB tests ended 07/2021.
2. There was no documentation of a TB screening or test on file for Staff #7.
3. Director confirmed there was no TB document in the file.Plan of Correction: I will the staff member to make an appointment to get the TB screening as soon as possible.
Standard #: 8VAC20-780-280-B Description: Based on observation, the center failed to ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In Classroom #1 there was a spray bottle marked "Mr. Clean" sitting on a half wall near the changing area.
2. Staff verified that the spray bottle marked "Mr. Clean" was not in a locked location that prevented access by the children.Plan of Correction: Staff immediately placed materials into a locked location. Administration will review the requirements regarding hazardous materials with staff.
