Comments:
This unannounced monitoring inspection was conducted on 6/14/19 from 10:00 am until 12:30 pm to review the religious exempt requirements. At the time of the inspection 44 children were present with eight staff. The sample size included five children's records and five staff's records. Children and staff were observed during outdoor play, indoor play, snack, transitions and behavioral management. Violations:
Standard #: 63.2(17)-1716-A-6
Description: Based on a review of the parent handbook and interview, the facility failed to provide a written statement to parents or guardians of the children in the center with information regarding the enrollment capacity of the facility.

Evidence:

1. The parent handbook did not contain information regarding the enrollment capacity.
2. The director stated the enrollment capacity is posted at the entrance but is not provided in writing to parents.

Plan of Correction: The enrollment capacity will be added to the parent handbook and provided to parents.

Standard #: 63.2(17)-1716-B-7
Description: Based on staff record review and interview, the facility failed to establish and implement a policy to ensure all staff are able to recognize the signs of child abuse and neglect.

Evidence:

1. The records for five staff were reviewed. The start date for staff 2 was 5/10/17. The certificate of completion of child abuse and neglect training is 8/29/17. The start date for staff 4 was 5/22/17. The certificate of completion of child abuse and neglect training is 9/1/17. Staff 5 was hired 1/18. There is no documentation in the staff's record on completing child abuse and neglect training.
2. The director stated the center has no policy to train staff as part of orientation in recognizing child abuse and neglect. All staff receive training in recognizing child abuse and neglect prior to each school year starting. The director stated Staff 5 did complete training in recognizing child abuse and neglect but not at hire.

Plan of Correction: Orientation for new staff will be changed to include training in recognizing child abuse and neglect.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review and interview, the facility failed to obtain fingerprint background checks prior to employment for staff hired after 1/22/18.

Evidence:
1. The records for five staff were reviewed. Staff 3's record documented the start date as 5/19/18. The fingerprint result in the staff file is dated 7/31/18.
2. The director verified the dates were correct based on the information she has available. The director was not working for the program at the time of staff 3's hire and fingerprint results.

Plan of Correction: The director will ensure all new potential hires will be required to complete fingerprint background checks prior to hire.
