Technical Assistance:
An exit interview was conducted with the director to discuss the results of today's inspection. Consultation was given regarding updated regulations in child/staff ratios (grouping of children), CPR, out of state searches, and accessing the department's website for memos, forms, and additional information. A copy of the updated regulations was given to the director.

Comments:
An unannounced monitoring inspection was conducted today, June 26, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at 2:25pm and concluded at approximately 4:40pm. Today 32 children (ages 2yrs-12yrs) were in attendance being supervised by twelve staff with the correct child/teacher ratio in each classroom. Administrative records, children's classrooms, restrooms, music room, worship center, playground, and common areas were inspected. See attached notice for the violations found during this inspection. Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on the inspection of administrative records, the required central registry background checks were not obtained. Evidence: During the inspection of five staff records, it was discovered that staff #2, #4, and #5 did not have a completed central registry background check in the record. This background check should have been completed on each staff employed by the center by the 30th day of employment.

Plan of Correction: Per administrator: I will have staff to complete the background checks so we can mail them off soon as possible.

Standard #: 63.2(17)-1716-A-6
Description: Based on the visual inspection of the facility and administrative records, the required written Code of Compliance Statements were not provided to parents.

Evidence:
During the visual inspection of the facility and review of administrative records, the following written Code of Compliance Statements were not provided or available to parents:

1. Written notice to parents or guardians of staff qualifications.
2. Written statement provided to parents or guardians and made available to the general public regarding physical facilities.
3. Written stated provided to parents or guardians and made available to the general public regarding enrollment capacity.
3. Written statement provided to parents or guardians and made available to the general public regarding health requirements for staff.
4. Written statement provided to parents or guardians and made available to the general public regarding possession of liability insurance.

Plan of Correction: Per administrator: I will have these statements added to our handbook or children's enrollment packet.

Standard #: 63.2(17)-1716-B-3
Description: Based on the inspection of administrative records, there were no staff present that have been trained to conduct daily simple health screenings of children. Evidence: During the inspection of five staff records, it was discovered there was no documentation that staff received training in daily health screenings. Religious exempt requirements stipulate that a staff should be able to conduct a daily simple health screening and exclusion of sick children by a person trained to perform such screenings.

Plan of Correction: Per administrator: We will have some staff to take the training soon as possible.

Standard #: 63.2(17)-1720.1-A
Description: Based on the inspection of administrative records, the required central registry background check updates were not obtained. Evidence: During the inspection of five staff records and interview with the director, it was discovered that staff #1 with a central registry background check dated 06/18/10 expired on 06/18/15; while staff #3 with a central registry background check dated 10/26/10 expired on 10/26/15 with no further updates in the record. These background checks should have been completed on each staff prior to the expiration of the previous central registry background check .

Plan of Correction: Per administrator: I will have staff to complete the background checks so we can mail them off soon as possible.
