Comments:
An unannounced monitoring inspection was conducted Tuesday, October 8, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 1:20 PM and concluded at approximately 4:15 PM. A total of 16 children were being supervised by two staff members.
Children were observed arriving at the center, participating in devotions, eating snack, working on homework and watching a movie. Five children?s records and five staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection. Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on review of five staff records, the the center did not obtain an individual's sworn statement or affirmation for one staff member.

Evidence:
The record for Staff #5 (hire date 09/19/19) did not contain documentation of a sworn statement or affirmation. Staff #5 completed the sworn statement during the inspection.

Plan of Correction: The sworn statement was completed during the inspection.

Standard #: 63.2(17)-1716-B-3
Description: Based on a review of records and interview, the center did not ensure there was at least one staff member present that had been trained to conduct daily simple health screenings to exclude children as necessary.
Evidence:
Five staff records were reviewed and did not contain documentation of daily simple health screening. Staff #3 stated the staff had not officially been trained to conduct daily simple health screenings.

Plan of Correction: Staff will be trained in daily health screening on 11/2/19.

Standard #: 63.2(17)-1716-B-4
Description: Based on the review of five children's records, the center did not establish and implement procedures to ensure that one child in the center is in compliance with the provisions of ? 32.1-46 regarding the immunization of children against certain diseases.
Evidence:
The record for Child #2 (first day of attendance 09/03/19) did not have documentation of immunizations. Paperwork presented to parents indicated a copy of the immunization record needs to be obtained prior to a child attending the center.

Plan of Correction: We will request a copy of the immunizations from the family again.
