An announced compliance monitoring inspection was conducted in conjunction with an initial subsidy health and safety inspection (SHSI) on January 9, 2020 from 10:00 a.m. to 4:30 p.m. with the assistant director. 13 children, ages 2 years through 5 years old were under the care and supervision of 2 staff. Center documents and six staff records were reviewed.

Standard #: 22VAC40-191-40-D-2 Description: Based on review of documentation and interview with staff, it could not be determined that the center obtained sworn statement or affirmation by the first day of employment or a central registry background check within 30 day of employment. Evidence: Staff #4, start date not recorded, had a sworn statement that was signed by the staff member but not dated. Staff #4, start date not recorded, began on or around July 15, 2019 per staff interviews and the central registry background check was dated 11/12/2019. Plan of Correction: Staff #4 will no longer be active as of 1/31/2020
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of documentation, the center did not obtain results of the national fingerprint background check by the first day of employment or service. Evidence: Staff #2 was observed being used to cover staff to children ratios in the classroom and did not have documentation of fingerprint background checks in the record. Plan of Correction: Staff #2 has been contacted to obtain fingerprint background check
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of documentation, it was determined that the center did not obtain documentation of an out of state central registry background check for staff that resided outside of Virginia in the past 5 years. Evidence: Staff # 6, start date 8/13/2019, indicated out of state residence in Ohio and the center did not have documentation of an out of state central registry check request. Plan of Correction: Staff #6 has obtained, printed and filed the out of state background check
