Comments:
A subsidy health and safety (SHSI) monitoring inspection was conducted to review compliance and to inspect the playground on February 19, 2020.

Violations:
Standard #: 22VAC40-665-640
Description: Based on observation, it was determined that the vendor did not maintain play areas free from obvious injury hazards including providing and maintaining cushioning material under playground equipment.
Evidence:
1. The white and green playset has a slide, two swings and a glider see-saw. The back use zone for the playset measured approximately 3 feet 8 inches from the left anchor bar and 3 feet 5 inches from the second bar to the fence. In the front use zone, the wooden play structure measured 6 feet 10 inches from the white and green playset. The swings on this playset require a use zone of six feet on all sides and a 10 feet 10 inch use zone in the front and back of the swing. Slides require six feet on all sides and 8 feet in front of the slide chute.
2. There is a large crack approximately 9 inches long in the bottom of the slide. The bottom rung of the ladder to access the slide is broken and hanging loose. There are two screws that extend beyond two thread lengths on both stabilizing bars (four total) that create entanglement and puncture hazards. The anchor hooks for the swings have large openings that exceed the required width.
3. The wooden play structure?s slide requires a use zone in the front of the slide of 7 feet 9 inches and measured 4 feet 2 inches from the fence. The swings attached to the wooden play structure require a use zone of 13 feet 8 inches and measured 6 feet 1.5 inches from the fence behind the swings and there are several pieces of equipment in the 13 feet in front of the swings.
4. There are three little tykes brand climbers, slides and climb on play equipment that do not have a sufficient use zone or resilient surfacing.
5. The resilient surfacing is not maintained and black landscaping plastic was observed in at least 6 different areas of the playground. The cushioning ability of the existing wood mulch and rubber mulch is unable to be determined as the materials are mixed together and there is no reference for the materials when mixed together to determine resilient surfacing/cushioning material.
6. There were five different areas of broken fence boards with sharp edges.
7. There were two screws and two nails exposed in the fencing that had more than two thread lengths exposed creating an entanglement and puncture hazard.

Plan of Correction: The green and white playground set will be removed. We are going to have all of the
old mulch removed and replace it with the rubber mulch six inches deep. We also plan
to reposition the playground around to give sufficient zone use and add sufficient
resilient surfaces. We will add the proper cushioning to be in compliance with the
program. We intend to have the fence repaired to meet the standards of the program.
I will be speaking with our executive director to determine when the playground will be
in full compliance with the subsidy program

Description: Based on review of the emergency preparedness plan, it was determined that not all required components were documented. Evidence: There was not documentation regarding procedures to address reuniting children with parent or an authorized person after shelter-in-place.

Plan of Correction: we have now added into our EPP procedures addressing procedures in-regards to
reuniting children with the parent or authorized person after shelter in place.

Description: Based on review of the emergency preparedness plan, it was determined that not all required components were documented. Evidence: There was not documentation regarding procedures to address reuniting children with parent or an authorized person after lock down procedures.

Plan of Correction: We have now added into our EPP procedures addressing procedures in-regards to
reuniting children with their parent or authorized person after lock down.
