An unannounced complaint investigation was initiated with the director and representative from the local agency on May 16, 2022 beginning at 1:10 PM and ending at 5:30 PM to investigate allegations regarding supervision, abuse and neglect, food services, and environmental hazards in the center. Another site visitation was conducted on May 17, 2022 beginning at 7:00 AM and ending at 8:32 AM. Children were observed arriving to the center. Between 7:00 AM and 8:00 AM, there were 10 children in the care of 1 staff. Children were also observed watching age appropriate educational videos, the children also colored, and eating breakfast. Center documentation was reviewed. The allegations regarding abuse and neglect were determined to be not valid. The allegations regarding supervision, food services, and hazards in the center were determined to be valid.

Standard #: 22.1-289.031-B-2 Complaint related: Yes Description: Based on observation, center staff failed to provide appropriate supervision of all children in care, including daily intake and dismissal procedures to ensure safety of children. Evidence: From approximately 7:00 to 8:00 am on May 17, 2022, the staff member left children unattended in the kitchen/lunch room area while she answered the front door for arriving children and staff. Standard #: 22.1-289.031-B-5 Complaint related: Yes Description: Based on observation, the center failed to ensure that all areas of the premises accessible to children was free from obvious hazards. Evidence: There was a large knife in the sink of the kitchen/lunch area which was accessible to children.
