As discussed the Conditional License is effective for 6 months, please submit the renewal application prior to the expiration date of your license. As discussed permission slips are required from parents for trips to the nearby playground.

An announced Initial inspection was conducted this date. No children in care at this time. Provider's own child present (2pts) along with one household member on site. Observation and inspection of the entire home was conducted to include the outside backyard. Programming area inspected, bathroom used by children, diapering and crib area were observed. Verification made regarding the two cribs on site comply with the Consumer Safety Product Commission. The home was found organized with ample supply of equipment and toys age appropriate for the requested age range. A total of 4 children's records, Provider and all household records were reviewed and found complete. Provider currently has 4 children enrolled at this time. No medications on site for the children enrolled at this time. This inspection began at 9:30am and ended at 12pm. Thank you for your cooperation today. Standard #: 22VAC40-111-430-B Description: Based on observation of the provider's home, pressure mounted gates were used at stair openings. Evidence: The gate located at the top of the stairs leading from the front door and to the second level, the gate located at the top and bottom of the stairs off the kitchen are all pressure mounted gates. Hard mounted gates are required. Plan of Correction: All three gates will be hard mounted by this weekend.
