Comments:
An unannounced monitoring inspection was conducted May 16 2019 between 11:00am and 12:00pm. There were 10 children in the direct care of the provider and 3 assistants. Children were observed playing outside, washing hands, eating lunch, and having circle time. The day care space is clean, organized, and offers plenty of space, toys, and books for the children. Standard #: 22VAC40-111-830-A
Description: Based on record review, the family day home did not document that evacuation procedures were practiced monthly. Evidence: The drill record form does not have a January evacuation drill documented. There is a shelter-in-place drilled documented as practiced in January.

Plan of Correction: Will make sure an evacuation drill is done every month.

Standard #: 22VAC40-111-180-A
Description: Based on record review, 1 of 3 staffing records reviewed did not have documentation of an updated report of tuberculosis every two years from the date of the first screening. Evidence: Staff #1 record documentation is dated February 16 2017.

Plan of Correction: I will complete it Monday of next week.
