Comments:
An unannounced monitoring inspection was conducted today between approximately 9:40am and 11:00am. 8 children (26pts) were present with 2 caregivers. Ratios were in compliance. The physical space, cribs, 4 children's records, caregiver records, attendance, evacuation drills and required posted documents were reviewed. No medications were on site. Children were observed playing outside. Areas of non-compliance are identified in the Violation Notice. For questions regarding this inspection, please contact stephanie.salvatori@dss.virginia.gov.

Violations:
Standard #: 22VAC40-111-830-A
Description: Based on records review and interview with the Caregiver, the monthly evacuation procedure was not practiced in the month of April. Evidence: The documentation showed, and the Caregiver stated, a Shelter-in-place drill was conducted in April instead of the evacuation drill.

Plan of Correction: I will do an evacuation every month and shelter-in-place drills will be in addition to the evacuation drills.

Standard #: 22VAC40-111-60-B
Description: Based on review of children's records, 2 out of 4 records did not have verification that the proof of identity had been seen. Evidence: The date and signature of the person viewing the proof of identification was not documented for Child #1 and Child #2.

Plan of Correction: I will review their information and complete the verification right away.
