An unannounced monitoring inspection was conducted on 6/6/19 from 9:35am until 11:15am. At the time of entrance, there were 11 children in care, for a total of 22 points, with the provider and one assistant. A sample of 5 children's records and 5 provider/assistant/household member records were reviewed. Children were observed participating in morning learning activities. Restroom and hand washing procedures were also observed. First aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills and required family day home postings were reviewed. The provider does not administer medication to children in care at this time. One violation in the area of administration is identified on the violation notice and was discussed with the provider during the exit interview.

Standard #: 22VAC40-111-60-B Description: Based on record review and interview, the provider did not ensure that children's records contain all the required information. Evidence: 1. The record for child 1 contains only one name, address and telephone number of a designated person to contact in case of an emergency if the parent cannot be reached, where two are required. 2. The provider confirmed that the record for child 1 contains only one emergency contact. Plan of Correction: The provider will obtain a second emergency contact for child 1 today at pick-up.
