Comments:
The Licensing Inspector for Michelle Borst family day home conducted an unannounced monitor inspection on 6-11-2019. Inspection started at 11:25 am & concluded at 1:45 PM. Upon arrival, the Provider present providing care for one 2 year old child. Records that were found out of compliance during the previous inspection were reviewed. Provider does not administer medication and does not transport children. An exit interview including review of Risk Assessment & areas of non-compliance found were reviewed with the Provider on 6/11/2019. Standard #: 22VAC40-111-770
Description: Based on inspection, the Provider failed to have required emergency supplies. EVIDENCE: A working battery-operated flashlight and a working portable battery-operated weather band radio was unavailable. Provider has not purchased replacement emergency supplies.

Plan of Correction: Provider will purchase emergency supplies.

Standard #: 22VAC40-111-210-A
Description: Based on inspection, the Provider failed to complete required annual training. EVIDENCE: Provider has not completed at least 16 hours of annual training within the past year. Provider provided documentation of 3 hours of the 16 hours of annual training.

Plan of Correction: Provider will seek online training opportunities & complete at least 13 hours of training. An additional 16 hours of training will be completed for the current year of annual training requirements.

Standard #: 22VAC40-111-460-C
Description: Based on inspection, the Provider failed to ensure all household pets (dogs or cats) are vaccinated for rabies and treated for fleas, ticks or worms as needed. EVIDENCE: There were two (2) dogs observed in the home. Vaccinations for Sasha expired 1/16/2019.

Plan of Correction: Provider will update rabies vaccinations for Sasha
